f a student’s parent, guardian, family member, or other individual wishes to obtain any of the student’s information (including, but not limited to, account balance, tuition payments due, class registration, etc.), the student must fill out and submit the Student Consent for Release of Records Form listing the names of the individuals to whom the student’s information may be released.
